What is an Alcohol/Drug Evaluation Assessment?
Informed Consent
An Alcohol and Drug assessment consists of a face-to-face clinical interview, using at least three (3) approved evaluation questionnaire instruments and a closing summary in which the results interpretations and conclusions of the evaluator are presented to you so that you will understand the recommendations of your assessment.
The evaluation assessment process is not a pass or fail process. There is no right or wrong answers, but to be accurately assessed, you need to be open and truthful in your response. Your information is protected by Federal (42 C.FR.Part 11) and State Law, which cannot be released without your written permission and with specific exceptions (child abuse, elder abuse, medical emergency, threats of harm to yourself or another person), may not be used to criminally prosecute you. The assessment interview will be concerned with your behavior regarding alcohol and/or other drugs use and the evaluator will assess the level and nature of your problem(s) with alcohol and/or other drug use. The process is intended to give you the right combination of services to help you to avoid similar troubles in the future.
The assessment process will take some time, about 1.5 to 2 hours. If you have problems with reading or comprehension, it may take longer. We will assist you as much as possible. Be sure to bring your reading glasses if you need them or an interpreter if needed.
You will be given the opportunity to sign a release of confidential information for the assessment results to be given a third party such as your attorney, the DA's office or the required court if needed. You should be aware of exactly what information will be released and to whom.
You have a right to understand the results of your evaluation assessment, and how it will impact you. These assessment results will determine a level of education, treatment or a combination of both to satisfy the court system you are dealing with or possibly the Department of Public Safety as a set of recommendations for reinstatement of your driving privileges.
Cost of the assessment is $175.00 which is due prior to the assessment. Anyone unable to pay the full amount may pay one hundred $100.00 to make the appointment and seventy-five $75.00 at the time of the appointment. The cost for education and counseling services recommended by the assessor may vary according to the agency used. You will be responsible for payment to the agency you select for such services.
If you should need to change your alcohol/drug assessment, contact this agency at least 24 hours prior to your scheduled appointment.
